6. Qualification card containing the last periodic assessment of your scientific work to date,
didactic, organizational and professional qualifications obtained at the University, and in the absence thereof
such, assessment of the head of the employing entity; in relation to candidates from outside the University
opinion of the last employer.

7. Declaration on the fulfillment of conditions in accordance with Article 113 of Act on Higher Education and Science of July 20th 2018

8. Declaration confirming PUMS to be primary employer.
